When verifying the identity of the applicant by mail, the applicant is required to submit identification documents.

 ※To open an account, you will need two of the following documents.

 

 

 

・National pension booklet

※Please submit the page with your name and date of birth as the front side and the page with your address as the back side.

※Please be sure to cover the basic pension number (symbols and numbers) with a post-it note or something.

※If you have a blue pension booklet, it cannot be used as an identification document as there is no space for your address to be entered.

・A copy of the certificate of residence
※If you have multiple pages, please submit all pages within 6 months of issuance.

・Certificate of seal registration
※If you have multiple pages, please submit all pages within 6 months of issuance.

・Disability certificate

 

For foreign citizenship

※Please submit two documents: one from the list below (ⅰ) and one from the list below (ⅱ).
Please submit two documents: one from among the following documents (i) and one from among the following documents (ii).
Example: If the name on the residence card is written in the alphabet and the name on the health insurance card is written in katakana
→The application will not be approved due to name mismatch.
