Identification documents to be submitted when changing your name or address

The following is an explanation of the identification documents you will be required to submit when you change your name or address.
Documents with and without a photo are different.

Foreign nationals may also use their Residence Card or Special Permanent Residence Certificate.

Notes

・Please submit documents that show your new name or new address.
・Please submit a photo of the original document (photocopies and scans are not acceptable).
・Please submit documents with an expiration date within 6 months of issuance.

「Documents with face photo」

Please submit one of the following

・Driver's license
※Please submit both the front and back of the driver's license.

・Passport

※Please submit the page showing the applicant's photograph and the page showing the holder's address.
※Passport applied for on or after February 4, 2020 (2020 passports) do not have a "Holder's Name" field, and therefore cannot be used as identification documents.

・My number card

※Please submit only the front side of your My number card.

【Documents without face photos】

Please submit the following two items.

※Please make sure that your address is written on the back of the card.

Please mask the symbol number, insurer number, and QR code with paper or other means before submitting the form.
Please note that masking other than the above may not be accepted.

・National pension handbook

Please submit the page with your name and date of birth on the front and the page with your address on the back, referring to the image below.

※Please be sure to cover the basic pension number (symbol and number) with a sticky note or the like before submitting the document.
※The blue pension book cannot be used as an identification document because it does not have a column for the address.

　Please submit the basic pension number after masking it with paper or other means.

・Copy of certificate of residence
If there are multiple pages, please submit all pages.

If your personal number or legal domicile number is included, please submit it after masking it with paper or other means.

・Certificate of Seal Registration
If there are multiple pages, please submit all pages.
